Forms listed below should be filled out online, printed and brought into the Pro-Health & Fitness facility the guest is going to use.
Adult guests
Minor guests, ages 13-17
Forms listed below must be completed by parent or guardian:
- Guest Registration
- Minor Hold Harmless
- This form provides parental consent allowing guests ages 13-17 to use Pro-Health & Fitness facilities if a parent is not in the facility at the time of use.
